Serves as a consistent care coordinator throughout the continuum of care assessing the physical, psychological and social needs of the patient. Assists in maintaining the highest possible degree of quality patient care by continually educating health care providers on proper and updated procedures.
Provides education to the patient/family regarding the disease processes and planned treatment/intervention. Provides instruction in the areas of diet, activity and medication as appropriate.
Demonstrates specific knowledge to prepare patients for breast special procedures and to assist them in making an informal decision regarding treatment.
Assess patients for procedures and provides coordination of care with Radiologist.
Demonstrates specific knowledge and skills to assist with breast special procedures and ensure proper sterile techniques by all personnel.
Provides emotional and educational support to patients.
Acts as liaison between Nursing and Radiology departments.
Provides leadership through involvement in committees, setting, and communicating nursing standards and promoting the professional image of nursing.
Participates in community educational developing an implementing teaching strategies for specific patient populations.
Establishes, in conjunction with the staff, teaching plans designed to educate the patient to adopt health-sustaining behaviors.
Assists the nursing staff in the development of their teaching skills.
Plans and directs individual staff members' orientation to the unit based on individual skills assessment. Is an effective preceptor.
In collaboration with Radiology and Nursing Administration develops, conducts and evaluates nursing education programs that meet the needs of the nursing unit and CentraState.
Utilizes effective teaching strategies and materials to provide staff education programs.
Current NJ RN license.
Minimum of five years nursing experience with previous teaching experience preferred.
Critical thinking and attention to detail a must.
Previous experience with breast health a plus.
Certified Breast Care Nurse (CBCN) certification preferred

Star and Barry Tobias Health Awareness Center - offers a variety of health education and fitness programs for adults and children, support groups, and health screenings to the community, schools, and businesses. Its Student Health Awareness Center provides programs specifically designed for students and faculty.

Applewood Estates – a lifecare retirement community that completed a $38-million expansion in 2007. The community now includes 380 independent-living, assisted-living and skilled-nursing apartments, and 20 independent-living cottages, along with a new promenade, performing arts center, formal dining room, and other enhancements.
Monmouth Crossing – Freehold's first assisted living community, providing 76 apartments (94 beds).
The Manor Health and Rehabilitation Center – a skilled-nursing facility licensed for 123 beds that accommodates short- and long-term stays.
SeniorsFirst - Community-outreach program that serves more than 14,000 members in Freehold and surrounding towns.
Family Medicine Center – a free-standing office about a quarter-mile west of the hospital providing community health services and serving as home to CentraState Medical Center's Family Medicine Residency Program.
CentraState Healthcare Foundation – CentraState’s charitable division. It recently completed a $10-million capital campaign to support the Star and Barry Tobias Ambulatory Campus.
Community Advisory Council - Under the auspices of the CentraState Healthcare Foundation, it identifies and targets the specific health needs of the various central New Jersey communities.
