Utilize Health is seeking a Patient Advocate I to join our growing team. We are looking for a passionate, detail-oriented clinician, who works as part of an interdisciplinary team, and is interested in helping patients navigate their health challenges throughout the healthcare continuum.
Most of our Patient Advocates work with patients remotely via telephonic and web-based platforms to deliver exceptional and timely service to our patients. Our approach is savvy, integrated care management, an approach with specialization in this vulnerable patient population. Through holistic care management, we identify and address patient health, social, and environmental issues that impact their health. Then, through careful, patient-centric goal setting, strategy implementation including targeted care coordination, we positively impact not only their health status, but their overall well-being.

JOB RESPONSIBILITIES OF THE PATIENT ADVOCATE
Coaching/ Educating patients on health and self-care topics
Providing guidance to patients and caregivers along the neurological continuum of care
Defining key social determinants of health and their impact on a patient’s health status
Identifying key red flag symptoms with their associated triggers and creating a mitigation plan
Providing care coordination to connect patients with appropriate services
Assisting with locating a primary care physician, physiatrist (or other specialty medical provider) as well as other community resources
Providing information regarding the latest research on a patient’s specific condition
Finding support groups, adaptive sports programs and personal care assistants

REQUIRED CERTIFICATIONS
To work as a Patient Advocate I, you must be licensed in one of the following areas:
Registered Nurse
Licensed Physical or Occupational Therapist (PT preferred)
EXPERIENCE
The following experience is REQUIRED:
8+ years of neuro or neuro-rehab experience in at least 2 different care settings
Has worked with patients who have neurological conditions (i.e. stroke, brain injury, spinal cord injury/disorder, MS, MD or CP)
The following experience is PREFERRED but not required:
Work experience in a physical medicine and rehabilitation setting
If nurse, CCM with neuro or neuro-rehab experience OR Certified Rehabilitation Registered Nurse (CRRN)
Neuro-developmental treatment (NDT) certification
Has coordinated care, including referrals, for this population of complex neuro patients
Experience with telephonic coaching and/or care management

Members with severe neurological conditions account for 2-4.5% of a health plan’s population and up to 12-24% of overall medical spend. These are members with stroke, brain injury, spinal cord injury/disorder, multiple sclerosis, muscular dystrophy and cerebral palsy. Utilize puts all our fees at risk and guarantees cost savings through our program. Our program is a blend of clinician services and proprietary digital tools. Utilize Health employs clinicians, called Patient Advocates, with a minimum of 8+ years of neurological care experience in at least 2+ care settings, along with Community Health Workers. Patient Advocates work with the patient inside their home, telephonically, and digitally to quickly optimize a patient’s care path. This optimization is accomplished through detailed care management, including: coordination of needed care, care path optimization using evidence-based clinical guidelines, patient/caregiver education and support, addressing social determinants of health and matching patients to the most appropriate service and... level of care at the right time and right place. Each patient is unique, and understanding our patients on a personal level allows us to tailor our approach to the individual.
