Home Health Care Coordinator LPN

Position Summary:The Care Coordinator is responsible for data collection, provides education consistent with identified problems and goals, assists with care management for consumers, participates in the development of care plans, and collaborates with community providers. The Care Coordinators will assist consumers with any needs related to a consumer's whole health.Duties and Responsibilities:* Participates in daily rounding meetings.* Assists with screening of potential clients for eligibility for behavioral health home services.* Completes health assessments on clients of the behavioral health home.* Collaborates with providers in the community who are involved with clients in the behavioral health home program. * Coordinates the clients care with community treatment providers. * Provides wellness activities to program participants.* Participates in treatment planning* Assists with required data collection on clients enrolled in the behavioral health home.* Participates in weekly staff meetings.* Assists with care transitioning of program participants* Provides and assists with linkages to community services* Conducts outreaches and engagement activities with program participants* Provides 24/7 on call support for consumers.* Assists with transporting consumers to needed appointments and services.* Meets or exceeds contracted commitments including budgeted volume of serviceMinimal Qualifications: Education: Graduate of accredited LPN programLicense Certification: Current and valid license to practice as Licensed Practical Nurse in NJ required. CPI, HSFAC, Blood Bourne pathogen required. Valid New Jersey Driver's License required.
