:: RN COMMITTED 12 WEEK ASSIGNMENT $40.00 HR W/ROTATING WEEKENDS DAYS & NIGHTS Coordinates the total nursing care for patients and their families or significant others. Provides leadership and works collaboratively with other nursing service personnel to maintain standards of practice and standards of nursing care. Assumes responsibility to assess and plan for the nursing care needs of patients and their families or significant others 24 hours per day, seven days per week. Implements the plan of care as assigned and evaluates the effectiveness of the care given. Assesses and documents initial data collection on patient admissions and subsequent reassessment at least once weekly according to policy and procedure. Documentation in the medical record according to policy and procedure. Develops a written individualized Initial Treatment Plan for the patient at the time of admission. Addresses discharge planning needs of the patient and family from the time of initial assessment throughout hospitalization. Documents and communicates effectiveness of nursing interventions and the patient's response to interventions in the medical record. Participates in development of the Master Treatment Plan utilizing appropriate nursing standards of care and practice. Attends and participates in the Multidisciplinary Treatment Team Meetings as required. Demonstrates competent nursing clinical skills, including thorough assessment of the patient's condition and nursing care needs, promoting total nursing care for the patient. Utilizes patient teaching opportunities. Provides, documents and includes in the Master Treatment Plan, patient and family education as the needs are assessed. Utilizes proper procedures in the application of special treatment procedures (restraint and seclusion) according to hospital policy and procedure and State of Kentucky Mental Health Laws. Demonstrates knowledge of patient rights according to policy and state law. Ensures patient rights are upheld and respected. Treats patients and families with respect. Facilitates prompt receipt of written diagnostic reports and communicates findings to the physician. Communicates at change of shift, accurate reports, verbal or written, of patients' behavior, status, and issues requiring follow up. Identifies abnormal diagnostic data promptly and notifies the attending physician as appropriate. Responds to emergency situations quickly and appropriately. Follows safety policies and procedures at all times. Reviews, provides feedback, and adds to documentation completed by the LPN or MHT if co-assigned for patient care. Assumes responsibility of charge nurse for a unit as assigned. Completes the patient care assignments based on staff skills and acuity/needs of the patient.
