Certified Coder This position is responsible for coding inpatient and outpatient medical records according to ICD-10-CM and CPT in keeping with AHA coding guidelines. Codes are assigned to all diagnostic and procedural statements as documented in each patient's medical record to determine the most accurate and optimal MS-DRG assignment. Essential Job Duties: - Accurately assigns ICD-10-CM diagnostic and CPT procedural codes in accordance with UHDDS definitions and coding guidelines as documented in the medical record with a 95% coding accuracy rate and 98% DRG accuracy.
- Sequences ICD-10-CM and CPT codes to assign the MS-DRG, which most accurately reflects the patient's acuity level.
- Effectively utilizes AHA Coding Guidelines and other reference sources available within the department.
- Accurately abstracts all codes and other assigned data into the SMS system.
- Consults the appropriate physician, verbally and/or in writing when conflicting or ambiguous information appears in the medical record.
- Produces a coding worksheet/attestation and places on medical record at time of coding.
- Serves as coding resource for other departments/providers.
