An exciting opportunity is available now for those interested in the quality of care for End Stage Renal Disease (ESRD) patients. Through implementation of quality improvement projects and activities, Health Services Advisory Group (HSAG) supports the Centers for Medicare & Medicaid Services (CMS) on improving the quality of life and quality of care for individuals receiving dialysis or a kidney transplant in the states of Arizona, Colorado, Nevada, New Mexico, Utah, and Wyoming. CMS calls this region Network 15.
Under general direction, this position will function with the Executive Director and Patient Services Director to assist in the development and implementation of patient services activities with special focus on patient and family engagement across all activities, provision of education to patients, families, and facilities, and grievance resolution. The position also functions with administrative, data, and quality improvement staff to meet and maintain all contractual obligations. Project deadlines may necessitate occasional extended work hours.

ESSENTIAL COMPETENCIES, DUTIES AND RESPONSIBILITIES
Collaborate with ESRD providers and provide technical assistance in managing challenging situations, conflict resolution, and accessing resources.
Assist with the resolution of patient and facility concerns and complaints.
Effectively communicate with ESRD beneficiaries, nephrologists, administrators, other ESRD staff, and CMS on case review processes and findings, as appropriate.
Develop educational offerings, including webinar presentations for facilities including conditions for coverage, patient and family engagement best practices and lessons learned, depression screening, and other requested topics.
Create and promote Network educational materials for patients, families, and other interested organizations through newsletters, the Network website, and other mechanisms.
Support the Network Patient Subject Matter Experts (SMEs) and the National Patient and Family Engagement (NPFE) Learning and Action Network (NPFE).
Develop and deliver appropriate communications to patients, facilities, and stakeholders.
Incorporates a continuous quality improvement philosophy as part of all activities.
Contributes to required administrative reports, e.g., monthly progress reports and annual reports.
Adheres to strict CMS and HSAG confidentiality and security guidelines.
Candidate must have a minimum of a Master’s degree in Social Work.
OTHER QUALIFICATIONS
Proficient in Microsoft Office systems.
Proficient in oral and written communication and interpersonal skills, including the ability to present and instruct through both written and verbal means.
ESRD or government contracting experience a plus.
Solid organizational and team building skills.
Proven ability to work on several projects simultaneously and in a fast-paced work environment.
Strong problem-solving skills and critical thinking skills.
Ability to work independently and as part of a team.
